Day Hab Coordinator/ QIDP (Millbrook, NY)

The Day Habilitation Coordinator/ QIDP is responsible to plan, organize, implement, and oversee the day to day operations, activities and services of the Cardinal Hayes Without Walls (WOW) Program.  This is a “hands-on” leadership position which provides supervision, assistance, guidance, and training to both the participants receiving services, as well as the Day Habilitation Specialist and Direct Support Professionals assigned to the program.

This is a full-time non-exempt position scheduled 35 hours per week, Monday-Friday (8:00am – 4:00pm).

 

Responsibilities:

  • Review and certify Habilitation Plans and Staff Action Plans
  • Review and certify monthly notes and billing accuracy
  • Develop community connections for the program
  • Supervise staff, maintain case records, payroll, petty cash and other duties

 

Qualifications:

  • Bachelor’s degree in human services or related field required
  • Minimum of one year’s experience working in this field
  • Driver’s License required
  • Proficiency in Microsoft Office required
  • Knowledge of Precision Care and OPWDD CHOICES preferred
