QA INVESTIGATOR – ON-CALL

We are a non-profit agency based in Millbrook, NY responsible for ensuring that all residents are cared for, that their needs are understood, that their preferences are followed, and that their rights are protected.

This position will perform impartial investigations, on and off site, of designated incidents according to regulatory and agency guidelines.  This is an On-Call position scheduled based on the needs of the Agency.

Qualifications:

  • Bachelor’s Degree in Human Services or related field preferred.
  • Two years’ experience working with individuals with developmental disabilities required.
  • Previous OPWDD incident management, quality, or risk management with demonstrated experience in Incident Management or Risk Management preferred.
  • Valid NYS Driver’s License required with access to a vehicle to transport self required.
  • Database management and/or development skills preferred.
  • Proficiency with current common computer applications, including but not limited to, Microsoft Office required.

Responsibilities:

  • Coordinate and maintain all notification, investigation, and reporting activities of the Agency to ensure compliance with applicable codes and regulations and to enhance the quality of care of all individuals.
  • Ability to write detailed investigative reports and to be able to effectively communicate and present findings to the Incident Review Committee.
  • Manage the Incident Report and Management Application (IRMA) system and the Web Submission of Investigation Reports (WSIR) Date Entry System.
  • Maintain on-going communication with all OPWDD, NYS Justice Center and other agency departments in order to facilitate the necessary services/ immediate protective action for each individual
