Physical Therapist Part Time (CHH- Millbrook)
Job Overview
We are seeking a dedicated and skilled Physical Therapist to join our dynamic healthcare team at Cardinal Hayes Home. The ideal candidate will be responsible for evaluating and treating patients with various physical conditions, developing personalized treatment plans, and helping patients achieve their rehabilitation goals. This role requires a strong understanding of physical therapy practices, including outpatient care, acute care, and specialized techniques such as trigger point therapy. This position is a Part Time Position – 5 hours per week.
Responsibilities :
- Conduct comprehensive evaluations of patients to assess their physical abilities and limitations.
- Develop individualized care plans that address specific patient needs and rehabilitation goals.
- Implement treatment programs that may include exercises, manual therapy, and modalities to improve patient outcomes.
- Provide education to patients and their families regarding treatment plans, exercises, and home health strategies.
- Monitor patient progress and adjust treatment plans as necessary to ensure optimal recovery.
- Collaborate with other healthcare professionals to coordinate care and discharge planning for patients.
- Maintain accurate documentation of patient evaluations, treatments, and progress in compliance with regulatory standards.
- Stay updated on the latest advancements in physical therapy practices, particularly in orthopedics and geriatrics.
- Evaluate wheelchair and orthotic needs, completing necessary medical paperwork associated with recommendations. Maintain and track equipment needs for residential department. Repair equipment as needed and/or assist on how to access needed repairs
Experience
- A degree in Physical Therapy from an accredited program is required.
- Current NYS licensure
- Experience in outpatient settings or acute care is preferred but not mandatory.
- Familiarity with systems related to discharge planning and care plans is advantageous.
- Knowledge of trigger point therapy techniques is a plus.
- Previous experience working with individuals with intellectual and developmental disabilities is a plus
