HABILITATION AIDE

We are a non-profit agency based in Millbrook, NY responsible for ensuring that all residents are cared for, that their needs are understood, that their preferences are followed, and that their rights are protected.
Provide transportation, daily care, training, supervision and support to individuals with developmental disabilities in a Day Habilitation Program. This includes implementing services based on person-centered Habilitation Plans, following behavior support plans, and providing hands-on physical care and/or assistance with daily living skills. Supervision and support include maintaining the safety of individuals at all times in the WOW program and the community and providing the assistance needed for them to accomplish their program goals. This position does not require medication certification and may be requested to work in the WATCH program if needed. This is a full-time position scheduled Monday through Friday, 8:00 a.m. – 3:30 p.m.
Qualifications:
- Demonstrates a good understanding of Cardinal Hayes values and culture
- Good Communication Skills (oral and written)
- Experience working with Individuals with DD preferred
- Valid NYS Driver’s License
- Ability to drive Minibus and/or Transit Van
- Proficiency with current common computer applications, including but not limited to, Microsoft Office is preferred
Responsibilities:
- Provides Physical Care in a manner which respects the dignity of the individual.
- Implements Services and Promotes the Development of Independence.
- Develops a Positive Relationship / Rapport with individuals receiving services.
- Provides daily transportation services for participants between residential and program sites
- Provides a clean and orderly Physical Environment.
- Provides Recreational and Social Development through individual activities suitable to individual preferences and one-on-one and group interactions.
- Requires Documentation that Accurately records individuals’ responses to services, daily outcomes and times spent in activities.
- Records attendance and participation for billing and payroll.
